Medical Forms and Immunizations for Spring Students
Due: December 10, 2025
For: All students under age 30 who are Undergraduate, Graduate, International students, and Residential students. Full and part time Health Science students, regardless of age, must submit the same documentation.
Undergraduate Student Requirements
- TB Questionnaire (student fills this questionnaire out on Patient Portal)
- Health History Form (student fills this questionnaire out on Patient Portal)
- Immunization Form (download Immunization Record Form, have your healthcare provider fill it in and upload it to the Patient Portal)
- Physical examination done within 13 months (submit a copy via Patient Portal)
Graduate Student Requirements

- Immunization Form (download Immunization Record Form, have your healthcare provider fill it in and upload it to the Patient Portal)
International Student Requirements
- TB Questionnaire (available on Patient Portal)
- Health History Form (available on Patient Portal)
- Immunization Form (download Immunization Record Form, have your healthcare provider fill it in and upload it to the Patient Portal).
- International students are required to have the same immunizations as someone living in the United States. Please make sure you discuss this with your doctor prior to your arrival.
- Physical examination done within 13 months (submit a copy via Patient Portal)
