For: New Undergraduate, Graduate, International students and part time Health Sciences majors
Undergraduate Student Requirements
- TB Questionnaire (available on Patient Portal)
- Health History Form (available on Patient Portal)
- Immunization Form (download Immunization Record Form, have your healthcare provider fill it in and upload it to the Patient Portal)
- Most Recent Physical (submit a copy via Patient Portal)
Graduate Student Requirements

COVID-19
If you have a fever or test positive for a respiratory virus such as FLU or COVID you are asked to take precautions to prevent spreading the virus. If you have severe symptoms, when possible, leave campus to recover at home. When this is not possible you should stay in your current housing assignment and attempt to socially distance from people as much as possible. Continue to stay away until you are fever free for 24 hours without the use of fever reducing medicines. For more information on preventing the spread of respiratory illnesses please review the CDC recommendations found here.
